- Expulsion, or permanent dismissal from the university
- Noting an academic integrity violation in the student's transcript

Procedures for reporting academic integrity violations
An instructor who suspects an academic integrity violation must fill out an Academic Integrity Violation Form Links to an external site., which is sent to the student, the instructor, the dean of the college responsible for the course, and the vice president of student services. The form must be submitted within seven days of when the violation occurred, prior to the sanctioning of the student. If a form isn't submitted, a student can appeal the determination of a violation.
Student Right of Appeal
Once a form is submitted, a student has seven days to respond to the notification and request a meeting with the instructor. If the student does not respond within seven days, or refuses to meet with the instructor, the sanctions in the violation form can be carried out and the student cannot appeal.
Resolution of Reported Violations
Attempted resolution of a reported academic integrity violation begins between the instructor and the student. If the student denies a violation, the issue can be escalated to the dean of the college and then to a hearing with the honor board, if further appeal is made. Each stage of the resolution process is governed by formalities and time frames. Hopefully the process is one you need never become familiar with. However, if you wish to learn more about the process in detail, please refer to the Article VI of the USU Student Code of Conduct Links to an external site., Sections 3-6.
